By providing my medical information I agree that this information may be shared with relevant club coaches & officials for my safety during training sessions. I consent to the provision of first aid treatment and in the event of medical treatment being required I give my consent that my information may be passed to the relevant treating medical attendees. We will not share your medical information with anyone, other than already stated, outside of the club, and will we not store your medical data after you have terminated your membership with Paddock Wood AC, your data will be stored securely in accordance with the current requirements for medical data.

I agree that the information I have provided is correct and that I am an amateur according to the definition of UK athletics. If elected by the club I agree to abide by the rules and regulations of the club & its facilities and act according to the appropriate club code of conduct on www.paddockwoodac.co.uk. By joining or renewing my membership of Paddock Wood Athletics Club I permit the club to disclose my information to appropriate team managers and club officials in order to inform you of club activities, training and competition opportunities and important club information, we will also share your details with regional and national athletic bodies as required, and as detailed below. By becoming a member or renewing your membership of Paddock Wood Athletic Club you will automatically be registered as a member of England athletics. We will provide England Athletics with your personal data which they will use to enable access to an online portal for you (called myAthletics). England Athletics will contact you to invite you to sign into and update your MyAthletics portal (where you can set and amend your privacy settings). I accept that photos of myself/my child may be taken and published on the Club's Website and/or social media channels or used to promote the club and its activities.

Gift Aid

I want to Gift Aid my donation/subscription and any donations/subscriptions I make in the future or have made in the past 4 years to Paddock Wood Athletic Club (We will claim gift aid on all memberships involving juniors, including family membership, or any separate donations to the club) I am a UK taxpayer and understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that tax year it is my responsibility to pay any difference. Please notify Paddock Wood Athletic Club if you want to cancel this declaration, change your name or home address, no longer pay sufficient tax on your income and/or capital gains. If you pay Income Tax at the higher or additional rate and want to receive the additional tax relief due to you, you must include all your Gift Aid donations on your Self-Assessment tax return or ask HM Revenue and Customs to adjust your tax code.

You can only compete in Kent Championships and/or represent Kent if you were born in Kent or have resided in Kent for 9 months AND you have not competed for another County or in another County Championships within the previous Competition Year (commencing 1st October).
